Nurses-
We have been working towards creating a Public Health Service Nursing coin to represent our mission and wide array of unique roles. This process is being lead by LCDR Cheryl Borden. In order to have this coin reflect the collective wisdom of our PHS Nurse Category, as well as, HHS Civil Service and Tribally employed nurses we have created a feedback process for you all to review some proposed coin designs (as discussed on the NPAC conf call 2/17/06) and fill out a short survey.

Instructions for submission are included in the Survey Forms ( in both pdf or the MS Word format). Survey may be filled out and emailed using the PDF version and utilizing the "submit by email" button on the top right of page. If you are unable to open the PDF version, you may open the WORD version, print, fill and fax.
